If you want a commercial product, Adobe makes a software package
called Captivate. At $799, it's a bit pricey for the casual user, but
it's toolset is pretty robust (it should be for that price).
If you want something open source (free) and relatively simple to use
on the PC, you can use Camstudio:

All you need is a microphone and you're in business.
